Job Duties and Responsibilities:

• Provide personal care to participants such as grooming, bathing, and dressing daily and as needed.
• Encourage participants to participate in hobbies, crafts and other activities.
• Record resident vital signs, weights and measurements, food consumption, and other information as assigned.
• Deal effectively with participants, families and staff in a professional manner.
• Attend required in-service education programs to meet state and facility educational requirements.
• Assist in planning, organizing and implementing group and individual activities based on participants’ individual needs as identified in participants’ health care plans.
• Assist in community planning related to the interests of the facility and the services and needs of the participant and family as directed by the Adult Day Services Director.
